If your insurance coverage is not approved through The Lactation Network or you prefer not to submit a claim to your insurance company, you have the option to self pay. Please complete the form below, and we will contact you to schedule a consultation.

SUPERBILL: We can create a superbill for you to submit to your insurance or other potential coverage you may have.
